This is the online edition of “Principles and
Practice of Medical Therapy in Pregnancy” which, over three editions, has become the most highly
regarded hard copy textbook on the management of medical diseases during
pregnancy. After three highly successful printed editions, there was consensus
that the future of this unique text had to be adjusted to the electronic
revolution that had swiped the academic world of publishing. What is being
presented here is the first step of this adjustment.
Instead of a 4th printed
edition, we have decided to present this comprehensive textbook in a constantly
updated electronic format. This decision was not an easy one. It is always
difficult to say good-bye to something that has been familiar to all of us,
and has been the subject of our work for over 20 years. Yet, repeated analyses
of this enormously detailed book project came all to the same conclusion: to
publish a text book of this size and this detail in hard cover, simply no longer
makes sense. Instead, if we wanted to remain true to our mission of providing
accurate and timely information on the management of medical and surgical
problems during pregnancy, we had to switch to an electronic format.
Consequently,
there will be no 4th edition of the book but a digital record of the
3rd edition which, on an ongoing basis, through a cooperative effort of authors
and a central editorial office in Chicago, will undergo constant updates.
In other words, not only the 4th edition is currently under way but, at the
same time, the 5th, 6th, etc., all concomitantly or in sequence, as the subject
matter requires. We anticipate at least one substantial revision of most of
the over 200 chapters by the first year anniversary of our initial on-line
venture. However, as individual topics may require, some chapters may, indeed,
have undergone more than one revision by then.
As with the printed version
of this text, each chapter will undergo a detailed peer review process before
publication. Norbert Gleicher MD, who has been the principle editor of this
textbook through its three editions, will supervise this peer-review process.
He is currently a Visiting Professor of Obstetrics and Gynecology at Yale
University School of Medicine in New Heaven, Connecticut and Medical Director
of The Center for Human Reproduction in New York City. The material published
as part of this electronic publication process will, therefore, be as authoritative
as the printed word was before. In addition, however, there will now also
be an opportunity for the reader to comment. Indeed, we expect this web address
not only to become a source for authoritative opinions on the diagnosis and
management of medical, surgical and obstetrical complications of pregnancy
but also a communal site for open exchanges between experts, practitioners
and, at times, involving the public as well.
Medical management in pregnancy has always been a difficult area of
practice to address. Pregnant women are, of course, principally, and most of the
time, under the care of obstetricians. On rare occasions family practitioners and
midwifes manage their pregnancies. When complications arise, obstetricians with
subspecialty in maternal-fetal medicine (perinatology) are frequently consulted
though even they, in most cases, obtain additional specialty consultations from
internal medicine sub-specialists and surgeons when pregnancy complications
appear to involve medical or surgical conditions. This is because obstetricians
usually have only limited expertise in those areas.
As obstetricians lack
knowledge in general medicine, its subspecialty and in surgical areas, so do
internists and surgeons lack knowledge about pregnancy and how it can affect
clinical presentations, their course and about the very specific treatments
needs of various medical and surgical conditions during pregnancy. Consequently,
their management has remained as complex as ever and, unfortunately, not
infrequently fails to live up to highest standards.
The principle mission of
this textbook has always been to serve as one authoritative source to all the
various communities of practitioners who have medical management responsibility
for the pregnant patient. This book historically sold over 70 percent of its
copies to the internal medicine community, approximately 25 percent to the
obstetrical community and the rest to a spattering of other interested parties.
Our assumption is that, at least initially, the electronic version of the book
will find a similar formula of distribution. One of the principle consequences
of the Internet has, however, been the “democratization” of information. As
knowledge has become so much easier to access, information finds much wider
distribution. We, therefore, expect a significant additional audience for this
textbook in the lay public, which has taken it upon itself to become more
informed and participatory in their medical treatments. Their participation is
very much welcomed and we are strongly motivated to make them an integral part
of the communal nature this website strives to become. Indeed, our goal will
only be reached if both physicians and the public will consider
www.medicaltherapyinpregnancy as their principle source of authoritative
information about medical, surgical and obstetrical complications in pregnancy.
By
going to an electronic format, we are also pleased to introduce ourselves
as the 3rd publisher of this textbook. After Plenum Medical and Appleton & Lange,
it is now TM-Publishing, a fully owned subsidiary of Trebron
Management, that has become the official publisher
of this classic text. We are extremely proud of having been given this
opportunity and are looking forward to a very rewarding relationship
with the editor, Dr. Gleicher, the many authors who will be contributing
